What to bring:
- Please bring a valid referral from either your GP or Specialist.
- Any relevant scans or x-rays (CT / MRI)
- Any previous hearing tests or balance test
- Any previous blood tests, allergy tests or sleep study reports
- Medicare Card
- Private Health care card
- List of current medications or allergies
Please advise our staff when booking the appointment if yourself or your patient require an interpreter as we are happy to organise this for you.
